Information and Precautions

We know these are challenging times for you and your loved ones. Like you, our utmost concern is for the health and well-being of each of our patients and to ensure our staff is healthy and able to continue to care for patients. With that being said, we continue to take all necessary cautionary measures and are strictly adhering to and implementing all CDC recommendations and guidelines. We continue to also work with local and state agencies as we fight Covid19. During this time we also recognize how important it is to keep the lines of communication open to give you peace of mind while we continue to meet the care needs of your loved ones while at our facility. As such, the facility will be providing updates of new developments as they take place on this page to keep you updated.

2021-22 Updates

March 4, 2022 As visitation is starting next week, I just wanted to clarify something regarding our outdoor/window visitation which you are more than welcome to continue to do with your loved ones here within the facility. Considering COVID-19 is still prevalent in the world, and its mode of transmission is airborne, when doing an outdoor/window visit we still require that you (the visitor) wear a mask. Any kind of mask will suffice. And regardless of Vaccination/Booster status, we are still asking that you wear a mask. You do not need to check-in or test or provide a negative test for outdoor/window visits and there is no time limit, but we do ask that you continue to follow the mask mandate to protect your loved ones and the other residents and staff that work here at Ontario Healthcare Center.

February 28, 2022

I am happy to send this update notifying you all that we are and have been COVID-free for both staff and residents over the last 2 weeks. We also closed down our red zone this past Wednesday, February 23, and do not have any active COVID-positive residents or staff.

We are still continuing to follow all the recommendations and guidelines for testing and vaccination/boosters given to us by Public Health, CDPH, and the CDC, and we will continue to communicate with them all of our needs and other issues that do arise as we continue to combat this pandemic.

More good news is that we will also start to open up to indoor visitations again starting next Monday, March 7. With us opening back up to visitation, we do still need to adhere to the guidelines to continue to protect the facility, the residents, and our staff from any exposure to COVID. Visitation will still need to be pre-scheduled like they were before with Diane, our Activities Director, or a facility designee in the event she is not in the facility. Visitors who are coming into the facility to visit with a resident, regardless of vaccination/booster status, will still need to have a negative PCR/Rapid test (with proof) within 48 hours of your visit. If a rapid test is the preferred method, it can be brought to the facility and done with an employee as a witness to the test and its negative result. Once cleared to come into the facility, all visitors must be screened with a temperature check and a questionnaire. Visitation will be somewhat monitored and all visitors must be checked out. We will not allow visitors into the facility to walk around the facility unattended. Visitation will again run on a daily basis from 9 AM to 4 PM and be 45 minutes in length. We are also going to limit the number of visitors per patient to 2 people at a time.

Thank you for your understanding and for trying to help us follow the regulations. It’s been a tough couple of years and I know that it’s been an inconvenience especially when trying to see and check on your loved ones. We don’t want to limit that any more than we need to and hope that one day soon things can go back to a regular norm as opposed to this new norm. We want to do what’s best and what’s right for our facility, residents, and staff.

February 10, 2022

Since last weeks update, we have had another 4 patients test positive via PCR testing and 2 via Rapid Testing. Currently inhouse we have 20 positive COVID patients in a Red Zone who are being monitored for signs and symptoms throughout the day. The majority of our patients are symptom-free and doing well. We are continuing to monitor the rest of the facility as exposures and considered to be in a Yellow Zone. Room changes were made to accommodate the Red Zone and try to prevent any more spread of COVID-19.

We also have 2 staff members/facility personnel who tested positive during the first round of testing this week. Those employees are quarantining and not currently working in the facility. We are continuing to follow the recommendations and guidelines from the CDC, CDPH, and Public Health as we are communicating/reporting any new positive cases.

Due to these new COVID positive cases, we are continuing to hold off on indoor visitations and will continue to update you as we monitor and hope to decrease the COVID activity within the facility with the continued push for infection control protocols, hand hygiene and proper use of PPE. You are all able to continue with outdoor/window visits as it works for those that are able.

February 4, 2022 

On Wednesday 2/2/2022, we did have to reopen our Red Zone due to Resident’s testing positive. Initially on Monday January 31st, we had 3 residents test positive. We were able to send them out to another facility accepting COVID positive patients. On Wednesday, 3 more residents tested positive so we opened up our Red Zone and brought back the 3 residents who went out to the other facility. Since that time we have also had another 6 residents test positive, so all in all we have 12 positive residents in the facility.

We also have 2 staff members who tested positive in the last 2 days. Those employees are quarantining and not currently working in the facility. We are continuing to follow the recommendations and guidelines from the CDC, CDPH, and Public Health as we are communicating/reporting any new positive cases.

We are quarantining and isolating staff and residents as needed to further prevent the spread of COVID-19 within the facility. Due to these new COVID positive cases, we are continuing to hold off on indoor visitations and will continue to update you as we monitor the COVID activity within the facility. You are all able to continue with outdoor/window visits as it works for those that are able.

January 26, 2022

As of yesterday 1/25/2022, we no longer have a COVID positive resident or a Red Zone in the facility. We do still have some residents and room in Yellow Zone for potential exposure to positive employees or their own symptoms, but we are continuing to monitor all patients for any signs and symptoms of COVID. But our goal is to continue to maintain a COVID-free facility, especially with our residents.

We do have 2 new positive employees as of Sunday and Monday, both of whom are CNAs. We are continuing to follow the recommendations and guidelines from the CDC, CDPH, and Public Health as we are communicating/reporting any new positive cases. We are also isolating or quarantining our employees and residents as needed with the hopes that help to stop the spread of COVID within the facility.

Due to the new positives and quarantining of our residents, we are still holding off on Visitation. We will definitely update you on the status of that when something changes.

January 20, 2022

In the last week we have had 6 new positive results turn up within the facility. 5 of those positives are employees and 1 of those positives being a patient. We rapid tested all the patients in the facility as we were preparing to tear our red zone down today and came across the one positive patient who is now in that red zone that did not get taken down.

We are going to be doing a PCR test to confirm her positive test and we have isolated the roommates that she had. We will continue to monitor them and the patient that tested positive.

Of the 5 staff that tested positive 2 were confirmed by PCR and 3 were confirmed with a rapid test. We are continuing to be in communication with and following the guidelines and recommendations of the CDC, CDPH and Public Health.

January 13, 2022

In the last week, we had 11 new positive results turn up here in the facility. 10 of those positives are employees and 1 is a patient. We rapid-tested all the patients in the facility today and they all turned up negative. We are opening up a red zone for the 1 positive resident and following the CDC and CDPH guidelines on isolating and quarantining and monitoring those that may have been exposed.

We are continuing to follow these CDC, CDPH, and Public Health guidelines and recommendations to ensure the safety and well-being of our facility, the staff, and residents. We are in constant communication with these entities regarding the positives within the facility.

We are continuing to hold on visitation due to the exposure our staff and residents may have, but as previously stated, the regulations have changed regarding visitation. When we do open up for visitation, all visitors regardless of vaccination status must present a Negative PCR test within 48-72 hours of their visit. This is including all of our vendors, doctors, transportation companies, etc. And upon entering the facility must still be screened and abide by the facilities PPE, social distancing, and isolation guidelines.

January 3, 2022

As most of you know we put a hold on visitations on Thursday 12/23/2021 due to potential COVID exposure within the facility. We are still on a visitation hold as we are in response testing due to a couple of employees and facility personnel who tested positive for COVID in the last week.

In the last week, we’ve had 4 employees/facility personnel test positive. So far based on the twice a week testing, no other employees or residents have tested positive.

We have initiated isolating and quarantining those staff members and the patients that may have been in contact with them and are continuing to follow all the CDC and CDPH guidelines and regulations. These regulations are constantly changing, and we do need to make sure we are in compliance.

Within these changes, there are some changes to the visitation policy that we will have to update regarding entry into the facility. Whether you are vaccinated or not, you will need to provide us with a negative test within 48-72 hours of your visit. All other regulations are the same regarding PPE, distancing, etc.


January 2021 On January 4th, we had a staff member test positive for COVID-19. This employee is home under quarantine. The facility has notified Public Health of this new development and are working jointly with Public Health to ensure the safety of its residents and employees.

December 26, 2020

This past week the facility conducted a facility-wide testing of its residents and staff in an effort to try and stop the spread of COVID-9. During the course of the testing process, the facility was notified that 11 employees and 7 residents tested positive. The employees are now home and under quarantine. The facility has notified Public Health of this new development and are working jointly with Public Health to ensure the safety of its residents and employees.


December 14, 2020

This past weekend the facility conducted a facility-wide testing of its residents and staff in an effort to try and stop the spread of Covid19. During the course of the testing process, the facility was notified that 12 employees and 31 residents tested positive . The employees are now home and under quarantine. The facility has notified Public Health of this new development and are working jointly with Public Health to ensure the safety of its residents and employees.


December 12, 2020 This week the facility conducted facility-wide testing of its residents and staff in an effort to try and stop the spread of Covid19. During the course of the testing process, the facility was notified that three employees tested positive. The employees are now home and under quarantine. The facility has notified Public Health of this new development and is working jointly with Public Health to ensure the safety of its residents and employees.

During the course of the testing process, the facility was notified that two employees tested positive. The employees are now home and under quarantine. The facility has notified Public Health of this new development and is working jointly with Public Health to ensure the safety of its residents and employees.

August 26, 2020

A part-time employee just informed us that she was tested by her Primary Care Physician for COVID-19 and the result came up positive. It has been 17 days since the employee worked in our facility, and she is still at home and under quarantine. All other employees and all residents have been tested since that employee’s last day of work and no other person has tested positive.

The facility has notified Public Health of this new development and is working jointly with Public Health to ensure the safety of its residents and employees.

A response testing process has been initiated at the facility in an effort to stop the spread of COVID-19. The facility’s priority is the safety of both residents and staff. The California Department of Public Health (CDPH) and San Bernardino County Department of Health have both been notified. Currently, no new order has been issued by either department.


July 8, 2020

A response testing process has been initiated at the facility in an effort to stop the spread of COVID-19. The facility’s priority is the safety of both residents and staff. The California Department of Public Health (CDPH) and San Bernardino County Department of Public Health have both been notified. Currently, no new order has been issued by either department.
